Frequently Asked Questions

What are your clinic office hours?

Monday – Friday 8:00 am – 5:00 pm.  The office is closed from 12:00 pm – 1:00 pm for lunch.

What can I expect as a new patient?

Our front desk staff will ask you to fill out new patient registration forms.  We ask that new patients arrive 15 minutes before the scheduled appointment time to complete the paperwork.  These forms may also be accessed on our website at brookwoodfp.com under “New Patient Forms”.

What is your appointment cancellation policy?

If for any reason you are unable to keep your appointment, please notify our office as soon as possible.  This allows our office to schedule another patient who is also in need of medical care.  As stated in our policy, there will be a $25.00 charge for the next missed appointment not cancelled prior to the scheduled time of your visit. Our practice is committed to providing the best treatment to our patients.  Please help us serve you better by keeping your regularly scheduled appointment.

What do I need to bring to my appointment?

Please bring a current ID (valid driver’s license) and your insurance card.  If your insurance requires a co-pay, deductible, or co-insurance, please be prepared to pay that at the time of your visit.  In addition, it is helpful to write down a list of questions for your provider so that we can answer all your questions in the exam room.

What is the best way to make follow-up appointments?

You can call during office hours at 214-547-8300 or make your follow-up appointment when you check out at the front desk.

How do I get a refill on my prescription?

If you have refills remaining on your prescription, request the refill through your pharmacy.  Once requested, the provider will review your medical record before prescribing or refilling the medication.  Some medications require routine follow-up and you will need to schedule an office visit.  Your provider will let you know when to schedule your next appointment and typically arranges refill quantities to match the follow-up schedule.

When will I get my lab/x-ray results?

Lab test and X-ray results may take up to 5 business days.  Generally, we have the results in 48 hours.  If after 5 business days, you have not heard from our office, please call us at 214-547-8300.

What if I need a referral?

If you need a referral, please contact our office to see if you need to schedule an appointment with your provider prior to the referral being issued.  Your insurance company may require that your Primary Care Physician has seen you prior to being sent to a Specialist.  Our office number 214-547-8300.

Will my insurance cover the services?

Each insurance plan is very different; therefore, we recommend that you contact your insurance plan to determine if the anticipated services are a benefit of your individual plan.  We are only able to provide estimates for the office visit portion of your visit.  If the services are not covered, we can work with you to develop a payment plan.

Who can help me with a bill that I received?

Our billing department is available to help you.  Please contact our Billing Department at 214-547-8300, option # 6.

What I am responsible for at the time of the visit?

You are responsible for any co-pay, deductible, or co-insurance amounts at the time of the visit.  When the insurance pays your claim, you will be billed for any remaining balances that are the patient’s responsibility.

What if I need paperwork filled out by my physician?

You can bring your paperwork with you at the time of your office visit or drop it off at the front desk.  Depending on the type of form, it may take up to 7 business days to complete.  Some paperwork might require an appointment with your provider for completion.
